We are proud to provide a state-of-the-art facility for the highest quality dental care available. It is one of our top priorities to protect the well-being of our valued patients. For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards. We are confident that you will feel right at home in our office as we welcome all patients as if they were family.

We are located in the Crestwood Professional Center South and across the street from Westgate Shopping Center.

Appointments

If you would like to make an appointment, please contact our office by phone or email. Our office facilitates communications and emergency calls, and appointments are available and welcome, especially for new patients seeking help.

*For our patients with emergency needs calling the office after hours, the office voicemail will instruct you how to page the doctor.

*For non-emergency calls, you can leave a message for the next business day of when we are open.

Financial

We participate with the following insurance plans and will gladly submit your claims (Any applicable co-pays are expected at the time of your visit):

• Anthem Blue Cross and Blue Shield
• Delta Premier
• United Concordia including “Tricare” for military dependants

We gladly accept all PPO insurance plans and will submit your claims as long as you are able to choose your own provider. 

You don’t need to put off treatment any longer, WE HAVE SEVERAL OPTIONS:

• We belong to the CARE CREDIT network which offers plans with as little as ZERO percent financing
• We accept all major credit cards
• Senior citizen adjustments!

Cancellation

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee will be charged if a patient does not show up for an appointment without sufficient notice. Please note that cancellations or rescheduling of appointments cannot be sent via email.
